Mid-week CHILDCARE: Servant Application

This form is intended for anyone in our fellowship who has a heart to serve our families and our children by watching and ministering to the children of our church during a mid-week church gathering. Examples of such gatherings include Tuesday night prayer meetings, Foundations class meetings, Friday Night Church Bible Study nights, etc.
